The program provides intensive one-on-one, in-home services that help families. The program will focus on the provision of non-medical services to eligible Medicaid Managed Care enrollees. The program provides services that include family and children support, case management, therapy, counseling, addressing food insecurities, health education, and other resource referral services.
Coordinating care and services to ensure that families receive the help they need in an organized and efficient manner. Case managers connect with families through home-visits, phone calls, and visits with families to the referred resources.
Providing mental health support to address emotional and psychological needs.
Assisting families in accessing nutritious food and addressing hunger issues.
Educating families about health-related topics to improve their overall knowledge and wellness.
Connecting families with additional resources and services in the community that can help them meet their needs. This approach emphasizes holistic care, ensuring that families receive not just medical treatment but also the support they need to thrive in other areas of their lives.
